How should I prepare for the first appointment?
If you have had any prior testing or treatment, you should bring your records to the first appointment. The following information is especially important to bring if you have it:
• Cycle day 3 or 4 FSH and estradiol level
• Hysterosalpingogram (please try to obtain a copy of the film from the radiology facility where you had this performed, in addition to the printed report)
• Any hormonal testing
• Most recent pap smear and/or cervical cultures
• Records of prior ovulation induction or IVF cycles
• Basal body temperature chart

• Semen analysis

Should your partner be present at the first appointment?
It is recommended that both partners come to the first appointment. If this is not possible, it is fine to come without your partner, as there will be other opportunities for either or both partners to meet with Dr. Bennett.
